While at the Sephiroth and Cloud battle, a peice of the wall actually hits Cloud when he's trying to get the sword out.

Loz threw the huge bench at Tifa and she didn't hit it

Loz was too early to throw the motorcycle which hit both Yazoo and Cloud

Kadaj was still on the phone arguing with one of the Turks and was too late to put the hand up which the montors attack Cloud

Cid didn't put his lance the right way which stabbed Cloud...you know where

Tifa tripped on the steps while trying to go to the phone

Cloud forgot to take out his sword while Reno just hit him with the metal bar
