When I was reading through this I was thinking to myself "Is Mandee just editing the local files and uploading it over the 'new' one?"

If that's the case, then 'unfixing' it might happen again.

I just renamed the file locally on the server. I guess Shlup had better redownload it to make sure her local copy on her Windows file system is using the correct case

I don't update the index page at all; I don't know how the old index page got moved instead of the fixed version.

If by "moved" you mean the server move, I'm pretty sure the album was broken for a long time before the server move too.
